Malaysia Medical Card Panel Hospitals: Can You Claim at Non-Panel Hospitals? How It Works, How to Search, and Claim Steps

If you hold a Malaysia medical card and go to a non-panel hospital, you can still claim, but you will not enjoy cashless admission. You must pay the full bill yourself first and then apply for reimbursement. Choosing a panel hospital allows direct settlement via a Guarantee Letter (GL), avoiding large upfront payments and complicated claims.

What Is a Medical Card Panel Hospital?

A Panel Hospital is a private medical facility that has signed a direct payment agreement with an insurance company or Takaful operator. When you are admitted to a hospital within this network, you can use cashless admission and do not need to pay the full eligible medical expenses upfront upon admission or discharge.

The core of cashless admission is the issuance and processing of the Guarantee Letter (GL):

  • Initial GL: After the specialist confirms that admission is needed, the hospital admission counter sends the doctor’s preliminary diagnosis and your medical card details to the insurer. Once the insurer verifies that the condition is covered and the policy is active, it issues the Initial GL. The hospital can then arrange the ward and treatment. You usually only need to pay a small administrative deposit.
  • Final GL: After treatment ends and the doctor signs the discharge papers, the hospital’s billing department compiles an itemised bill (medicines, tests, nursing, surgery, etc.) and sends it to the insurer. After review, the insurer issues the Final GL confirming the amount it will pay.

Note: People often confuse Panel Clinic (for outpatient care, common in company group medical benefits) with Panel Hospital. Personal hospitalisation medical cards mainly cover inpatient treatment, major surgery, or day surgery at panel hospitals. Most personal medical cards do not allow direct outpatient use at general clinics.

Why Choose a Panel Hospital When Claiming?

The main benefits of going to a panel hospital are:

  • Avoid large cash advances: Private hospital surgery, ICU, and ward fees can easily reach tens of thousands of ringgit. With a Guarantee Letter, you avoid financial pressure or delayed treatment.
  • Faster administrative processing: Direct system linkage between the insurer and hospital speeds up Initial GL issuance and reduces waiting time for admission and bed allocation.
  • Lower risk of excess or unreasonable charges: Panel hospitals follow contracted rates and medical fee guidelines, reducing the chance of claim deductions or disputes over “unreasonable charges.”

Panel Hospital vs Non-Panel Hospital Comparison

Aspect Panel Hospital Non-Panel Hospital
Admission Deposit Usually lower (approx. RM300–1,500; some may waive it) Higher deposit required (often RM1,000–3,000+ or estimated full amount)
Guarantee Letter (GL) Supports e-GL and cashless process No GL support; no cashless admission
Discharge Settlement Time Wait ~2–3 hours for Final GL review Can leave after paying the bill; must claim later yourself
Excess Charge Risk Protected by contracted rates; rare disputes Strictly limited to “reasonable and customary” charges; excess must be paid by you
Claim Method Hospital settles directly with insurer Pay full amount first, then submit documents for reimbursement (“Pay and Claim”)

Can You Claim at a Non-Panel Hospital?

Yes, you can claim at a non-panel hospital, but you cannot use cashless admission. You must use the Pay and Claim method.

Insurers will strictly review Reasonable and Customary Charges. They only reimburse based on the average market rate for the same treatment at similar hospitals in the same area. Any amount significantly above this will not be covered.

Emergency exception: In life-threatening situations (serious traffic accident, acute heart attack, severe stroke, heavy bleeding, etc.), go to the nearest hospital immediately. After stabilisation, you may request transfer to a panel hospital. Emergency treatment costs at non-panel hospitals are usually reimbursed after review of police or medical reports.

Pay and Claim Steps and Required Documents

  1. Settle the full bill and collect all original documents before leaving the hospital.
    Required documents typically include:

    • Original official receipts
    • Original itemised bill (detailed breakdown of medicines, tests, nursing, surgery, etc.)
    • Attending Physician’s Statement (APS)
    • Laboratory and imaging reports
    • Discharge summary
  2. Complete the insurer’s official claim form (download from website or app). Fill in your details and have the specialist doctor sign and stamp it.
  3. Submit within the time limit (usually 30–90 days after discharge). Late submission can lead to rejection. Many insurers now accept e-claim via app, but keep original documents for at least 6 months.
  4. Processing and payment: Insurers usually take 14–30 working days. After approval, the net amount (after deductible, co-payment, and excluded items) is credited to your bank account.

How Panel Hospitals Work in Malaysia

Panel networks are commercial partnerships between insurers (or Third Party Administrators) and private hospital groups. Major groups include KPJ Healthcare, IHH Healthcare (Pantai, Gleneagles), Sunway Medical Centre, Columbia Asia, and various independent specialist hospitals. Coverage varies by insurer. Some hospitals accept most major insurers; others are exclusive to specific companies. Some policies also have tier systems (e.g., Platinum/Gold) with benefits such as deposit-free admission.

Under Bank Negara Malaysia (BNM) Medical and Health Insurance/Takaful (MHIT) guidelines, co-payment features became mandatory from September 2024, and by 2026 co-payment and deductible mechanisms are widespread:

  • Panel hospitals: Often benefit from contracted rates and maximum out-of-pocket caps.
  • Non-panel hospitals: Higher out-of-pocket risk due to possible excess charges on top of any co-payment/deductible, plus no e-GL support.

How to Search for Panel Hospitals or Specialist Clinics

Use official channels for the most up-to-date list:

  • Insurer’s official app (Panel Locator) with GPS
  • Insurer’s website Healthcare Locator (search by state or specialty)
  • 24-hour hotline printed on the back of your physical or digital medical card
  • Hospital admission counter — show your MyKad and medical card for real-time verification

Frequently Asked Questions

Do I have to go to a panel hospital in an emergency?

No. Prioritise the nearest hospital. After stabilisation, you may transfer to a panel hospital. Emergency costs at non-panel hospitals can usually be claimed later.

Why do I still have to wait 2–3 hours at a panel hospital after the doctor issues the discharge notice?

The hospital is preparing the itemised bill and waiting for the insurer to issue the Final GL. This verification process typically takes 2–3 hours.

Can the doctor’s medical report fee be claimed?

Usually no. The fee for the Attending Physician’s Statement (typically RM60–150) is considered an administrative expense and is excluded under standard policy terms.

What if the hospital is panel but the specialist doctor is non-panel?

You may not get full cashless treatment. Hospital facility fees may be covered by GL, but the doctor’s fees might need to be paid first and claimed later. Always verify the doctor’s panel status at admission.

What if I am unconscious or don’t have the physical medical card in an emergency?

Family members can provide your MyKad number and the insurer’s name. The hospital can usually look up the policy. You can also show the digital medical card via the app. Even if registered as self-pay initially, you can convert to cashless within 24–48 hours by providing the medical card details.

Why do I still have to pay something at a panel hospital?

Common reasons: co-payment or deductible under BNM rules, non-covered items (companion bed, visitor meals, personal phone charges, etc.), or choosing a higher-grade ward that exceeds your policy limit.

Do insurers change the panel hospital list?

Yes. Agreements are reviewed periodically. Always check the latest list via the app or hotline before planned treatment.
