What is Room & Board (R&B)?

Room & Board (R&B) is a specific limit in Malaysian medical insurance (medical cards) that reimburses daily bed rental, three daily meals, and basic nursing care during hospitalisation. This limit directly sets the ward class you can occupy in a private hospital and is separate from items such as surgery and medication, which are usually covered as-charged.

R&B typically covers:

  • Daily bed rental (Room Rental): The basic daily charge for occupying a hospital bed.
  • Meals (Board): Nutritious meals provided by the hospital according to the doctor’s orders.
  • Basic nursing and ward services (General Nursing Care): Routine nurse rounds, vital sign monitoring, and similar daily care costs.

Malaysian medical cards commonly offer daily R&B limits such as RM150, RM200, RM300, RM400, and RM500. Higher limits usually mean higher annual premiums.

A common misconception is that “as-charged” coverage means everything is unlimited. In reality, surgical fees, anaesthetist fees, specialist visits, lab tests, and prescribed medicines are often covered as-charged, but daily room and meal costs are strictly capped by the R&B limit stated in the policy.

Therefore, the R&B tier affects not only privacy and comfort during recovery but also how the entire medical bill is settled.

Comparison of Private Hospital Ward Charges in Malaysia

Daily ward rates at Malaysian private hospitals vary by room type and location. Standard single rooms currently average RM250–RM380 per day, while twin rooms average RM150–RM220. With medical inflation exceeding 10% annually, older policies with a RM150 limit can now only cover multi-bedded wards in most private hospitals.

Ward Type Average Daily Rate (RM) Typical Features Recommended Medical Card R&B Limit
4-Bedded RM80 – RM120 4 beds sharing bathroom; busier with visitors RM100 – RM150
2-Bedded RM150 – RM220 2 people sharing private bathroom; curtain separation RM150 – RM200
Standard Single RM250 – RM380 Private bathroom, TV, sofa/foldable visitor chair; quiet RM250 – RM400
Deluxe / Suite RM400 – RM1,000+ Separate living area, microwave, butler service RM500+ or self-pay difference

Rates are higher in core metropolitan areas such as the Klang Valley (Selangor/Kuala Lumpur), Penang, and Johor Bahru, where standard single rooms often cost RM280–RM380. In secondary cities such as Ipoh, Seremban, or Malacca, standard single rooms may start around RM220–RM260.

Malaysia’s private medical inflation has long stayed at 10–15%. Rising costs for ward maintenance, staff salaries, and equipment have pushed up daily rates. Many people holding older policies with RM150 or RM200 limits now face significant coverage gaps when wanting a standard single room in the Klang Valley.

Most people prefer single rooms for three main reasons:

  1. Privacy and dignity — Own bathroom, easier for changing, washing, and wound care.
  2. Quiet rest and family support — Less noise from equipment, neighbouring patients, or visitors; space for family to stay.
  3. Lower cross-infection risk — Especially important during flu seasons, respiratory outbreaks, or after major surgery.

How Much Do You Pay When Upgrading a Room?

If you stay in a higher-class room that exceeds your medical card’s R&B limit, the amount you pay depends on whether the policy uses a simple “pay the difference” method or a “pro-ration penalty” (proportional co-payment) mechanism.

Method 1: Pay the Difference Only

You only pay the excess room charge out of pocket. Other eligible medical costs (surgery, tests, medicine) are still covered by the insurer as normal.

Formula:

Self-pay amount = (Actual daily room rate − Daily R&B limit) × Number of days

Example:

Mr. Tan’s daily R&B limit is RM200. He stays in a standard single room at RM300/day for 3 days. Surgery and medical bills total RM20,000.

Room difference he pays: (RM300 − RM200) × 3 = RM300.

The insurer still covers the RM20,000 medical bill. Total out-of-pocket: RM300.

Method 2: Pro-ration Penalty (Beware)

Some traditional policies apply a pro-ration or co-payment penalty if you upgrade beyond your limit. The insurer may not only refuse the excess room charge but also require you to share 10–20% of the entire medical bill.

Formula:

Self-pay amount = Room difference + (Eligible medical total × co-payment percentage, e.g. 10%)

Example (same as above, with 10% penalty):

  • Room difference: RM300
  • Penalty on medical bill: RM20,000 × 10% = RM2,000
  • Total out-of-pocket: RM2,300

This can be several times higher than simply paying the room difference.

Voluntary Upgrade vs Forced Upgrade Due to Full Rooms

  • Voluntary upgrade: You choose a more luxurious room yourself. You bear the difference and any pro-ration penalty.
  • Involuntary upgrade (full rooms): The hospital has no available rooms that match your limit and places you in a higher room for medical reasons.

If this happens, ask the admission counter to clearly note on the Guarantee Letter (GL) or medical records:

“Eligible room fully occupied / Involuntary upgrade.”

With this written proof, you can apply for the policy’s Compassionate Clause to try to waive the pro-ration penalty on the whole bill, so you only pay the room difference.

Note: Choosing a room above your limit also usually increases the admission deposit. Hospitals often require an extra RM1,000–RM3,000 cash/credit card deposit when they see you have selected a higher room.

4 Key Factors When Choosing R&B Limits

  1. Hospital pricing in your area
Base the limit on private panel hospitals near your home or workplace. In the Klang Valley, Penang, or Johor Bahru, standard single rooms often start above RM280, so a daily limit of RM300 or higher is recommended to avoid paying out of pocket.
  2. Buffer against medical inflation
Fixed limits lose purchasing power over time. Choose a higher limit now or a plan that automatically increases the R&B limit periodically.
  3. Complementarity with basic plans (MediAsas / Base MHIT)
Bank Negara Malaysia and the Ministry of Health’s standardised basic medical plans (MediAsas) usually cover multi-bedded wards and serve as an affordable foundation. For private single rooms and higher comfort, a commercial medical card is more suitable.
  4. Policy terms — Prefer transparent plans without pro-ration penalties
Check the upgrade and co-payment clauses carefully. Some modern digital medical cards offer R&B limits that cover standard single rooms (e.g. RM300/day) and do not apply pro-ration penalties. Even if you upgrade further, you only pay the room difference, while eligible treatment costs remain fully covered.

Frequently Asked Questions

If the standard single room is full and I am placed in a deluxe suite, what documents do I need to avoid the pro-ration penalty?

Request a written note from the admission counter on the Guarantee Letter stating that all eligible single rooms are fully occupied and the upgrade was involuntary. Submit this with the claim to apply for the Compassionate Clause.

Does ICU (Intensive Care Unit) fall under the daily R&B limit?

Usually no. Most modern medical cards treat ICU or HDU as a separate benefit, often covered as-charged (subject to annual day limits, e.g. 150–200 days).

Does day surgery (no overnight stay) use up the R&B limit?

Normally no. Observation or day-bed charges are typically covered under Day Surgery Benefits or hospital miscellaneous expenses (if the policy covers day surgery and the procedure is medically necessary).

Will the hospital ask for a higher deposit if I choose a room above my R&B limit?

Yes. Hospitals usually require an extra RM1,000–RM3,000 deposit when they anticipate you will have out-of-pocket room charges.

If I want to increase my R&B limit later, do I need a new medical underwriting?

Usually yes. Increasing the limit is treated as an upgrade, so the insurer may require a new health declaration, possible loading, exclusions, or medical check-up. The new higher portion may also restart the waiting period. It is generally safer to set a sufficient limit while you are young and healthy.
