On the contrary, if you can accept sharing a twin or 4-bedded room, an RM150 or RM200 limit may be sufficient.
  2. Premium Budget The R&B limit of a medical card is directly proportional to the premium. The higher the R&B limit, the higher the annual premium payable, or the insurance charge deducted in an Investment-Linked Policy. It is recommended to strike a balance between protection needs and long-term payment ability.
  3. Future Medical Inflation Rate As medical costs rise year by year, today’s RM250 single room may exceed RM350 in 5 to 10 years. Therefore, if financially feasible, it is advisable to set the R&B threshold slightly higher (e.g., directly choose RM200–RM300 range) as a buffer against future inflation.

R&B Upgrade: How Much Do You Need to Top Up for Upgrading Room?

If your medical card R&B is RM150 but you choose a RM250 single room during hospitalization, besides paying the room difference out of pocket, you may also face hidden co-payment penalties in the medical card.

The most basic situation is paying the difference: Suppose you are hospitalized for 5 days with a daily room difference of RM100, you will need to pay RM500 (RM100 × 5 days) out of pocket upon discharge.

However, what requires more attention is the co-payment/co-insurance penalty: Many medical card terms stipulate that if the cardholder “voluntarily exceeds the R&B limit” to upgrade the room, in addition to paying the room difference, they must also bear a certain percentage (usually 10%–20%) of the entire hospitalization bill (including expensive surgery fees, medicine, anesthesia, etc.). Therefore, before purchasing or admitting, be sure to check your policy terms with the insurance agent to confirm whether upgrading the room only requires paying the difference or triggers percentage-based charges on the total bill.

Bank Negara Malaysia (BNM) has implemented new guidelines for Medical and Health Insurance and Takaful (MHIT), with the most notable being the reform of the co-payment mechanism.

Under BNM’s new rules, insurers must offer medical card options that include co-payment (minimum 5% or specified deductible), which usually come with lower premiums. However, if you purchase such a co-payment medical card, you must bear a certain percentage of medical expenses regardless of whether your R&B limit is exceeded during hospitalization.

In addition, many people are told during sales that the medical card is “As-charged (full reimbursement)”.
